To schedule a reservation with us, please call 323-3944. If you are not able to keep your reservation, we ask that you give us at least 48 hours advance notice so that another patient can be seen by the dentist or hygienist. That way you can avoid charges ($50 per hour) for missed reservations.
Even though we schedule carefully, emergencies can cause unexpected delays.
We realize your time is as valuable as ours, so, if your reservation time
will be affected, we'll try to notify you as soon as we can.
Please be sure we have your home, work and cell telephone numbers.
Your First Visit
On your first visit, please bring with you your completed forms and any medications you are currently taking. (If you didn't receive the forms, please come 15 minutes early to fill them out.) The more we know, the better we can care for you.
We'll also need information about your
insurance coverage (if we didn't get that
information over the phone). Should any of this
information change between visits, please let us
know so we can update your records.

we can from you over the phone so we can
decide how long we'll need to see you. Let us
know if you need medications prior to having
